Davis Vision Provider Lookup Instructions
When searching for an in-network provider on the Davis Vision website at www.davisvision.com, please be sure to follow click on the click on the “Member Log In” oval button on the top right hand side of the web page and enter 9013 under the "Client Code:" field and click submit to acccess the Davis Vision in-network providers specific to the Jefferson-Lewis Healthcare Plan. Jefferson-Lewis Healthcare Plan members should not use the “Find a Provider” oval button on the middle of the Davis Vision homepage on their the website. Please note that only members of the POS plan receive a vision benefit.
