Davis Vision Provider Lookup Instructions
When searching for an in-network provider on the Davis Vision website at www.davisvision.com, please be sure to click on the “Member” menu tab on the left hand side of the web page and enter client code number 9013 at the bottom right of the next screen. Jefferson-Lewis Healthcare Plan members should not use the “Find a Provider” button on the top left corner of the website. Please note that only members of the POS plan receive a vision benefit.
